Youth Offender Care Coordination (YOCC)

The Youth Offender Care Coordination (YOCC) program has been created to effectively respond to the social, emotional, behavioral and educational needs of court-involved juveniles, specifically, repeat juvenile offenders with a history of weapon charges who reside in the City of Hartford and have current Juvenile Court involvement, including Juvenile Probation Supervision.

Referrals are provided to the Department of Families, Children, and Youth (DFCY) by the State of Connecticut Judicial Branch, Court Support Services Division Hartford Juvenile Probation Services (JBCSSD-HJPS) and member organizations after identifying high-risk, high-need post-adjudication juveniles under probation supervision.

Understanding that information related to the participants involved in this program is sensitive and confidential, identifiable information will be shared only with the contracted members of the YOCC program and will be used to support improved outcomes for the identified youth by providing them with a range of coordinated wrap-around support services in the community. Identifiable information shared within the YOCC program shall remain confidential and will not be used for any purpose outside the scope of services.

Five agencies have been tasked with assisting DFCY and JBCSSD-HJPS in this program. Learn more about them below.

  2. Our Piece of the Pie (OPP, https://opp.org/), who’s mission is to empower “youth with the key competencies needed to overcome barriers and succeed in education and employment.”
  3. Roca (https://rocainc.org/), who’s “mission is to be a relentless force in disrupting incarceration, poverty, and racism by engaging the young adults, police, and systems at the center of urban violence in relationships to address trauma, find hope, and drive change.”
  4. Second Chance Reentry Initiative Program (SCRIP, https://www.scripconnect.org/), who’s mission is to help marginalized individuals navigate the challenges and constraints in society to improve their lives and become successful in their career endeavors.”
  5. Center for Children’s Advocacy (CCA, https://cca-ct.org/), who’s mission is to fight “for the legal rights of Connecticut’s most vulnerable children.”
